INFORMATION TO CANDIDATES
(i) Scan copies of Mark Sheet, Provisional Certificate of HSLC/Equivalent (Class-X), (Class XII) examination and Admit card for age proof, NEET Score Card etc. shall be enclosed.
(ii) Certificate of character from the Principal of the School last attended duly attested by a Gazetted Officer shall be enclosed.
(iv) Domicile/Residential Certificate from the Deputy Commissioner or an Officer authorized by D.C. of the District to which the candidate belongs shall be enclosed.
(v) In case of Schedule Caste, Schedule Tribes or OBC candidates, a certificate on that behalf from the District Magistrate or an Officer authorized by D.C. of the District to which the candidate belongs shall be enclosed.
(vi) Health/Medical Fitness Certificate issued by a Medical Officer of the State/Central Government shall be enclosed.
(vii) Submission of Application fee Rs. 1,200/- (Rupees one thousand two hundred) only for General, In-Service Candidates & OBC Candidates and Rs. 700/- (Rupees eight hundred) only for SC/ST/Person with Disabilities (PwD) Candidates.
